Entry form for new patients

If you are going to visit us for the first time, here is the form you can get at your first visit to fill out. Questions concern your health status and are important to your treatment. We will also ask you for any changes regarding your health during the next visits as your health condition or the medications you take can affect the condition of the oral cavity or the course of treatment. You can also print your form here to bring it to us.

If you can not come.

If for any reason you can not come to the agreed treatment date, please let us know as soon as possible by telephone or email so we can offer your term to another patient. If you do not show up for a dentist or dental hygienist without excuse, we will charge you for non-observance.

Term of acute treatment

If you have acute pain, of course we will try to treat you as soon as possible, but if you do not go to regular annual visits or if you are a new patient, a fee may be charged for the priority date.

Possible to send regular notifications to your dental treatment

We, at your request, send a reminder at regular intervals by e-mail or SMS that it is time for your preventive or dental hygiene. Usually after 6 or 12 months. Then you can negotiate by phone or email the date of your visit.
